The K25-MS-T is Yamada's fluid-section service kit for the the matching Yamada pump — the wetted elastomers that wear out in normal service and are replaced together at rebuild.
Falling flow and lost prime are the usual signs. Most plants running these continuously keep a kit on the shelf rather than ordering one after the line is already down.
